New and Revised CPT Codes for 2011

The 2011 Current Procedural Terminology (CPT) book published by the American Medical Association contains several new level one codes of significance to ophthalmology. They are:

  • 92132 Scanning computerized ophthalmic diagnostic imaging, anterior segment, with interpretation and report, unilateral or bilateral
  • 92133 Scanning computerized ophthalmic diagnostic imaging, posterior segment, with interpretation and report, unilateral or bilateral; optic nerve
  • 92134 Scanning computerized ophthalmic diagnostic imaging, posterior segment, with interpretation and report, unilateral or bilateral; retina

The new code 92132 replaces Category III code 0187T. Additional instructions associated with these codes state:

  • Do not report 92133 and 92134 at the same patient encounter
  • 92135 has been deleted

The Ophthalmoscopy section of CPT includes the following new codes:

  • 92227 Remote imaging for detection of retinal disease (eg, retinopathy in a patient with diabetes) with analysis and report under physician supervision, unilateral or bilateral
  • 92228 Remote imaging for monitoring and management of active retinal disease (eg, diabetic retinopathy) with physician review, interpretation and report, unilateral or bilateral

Additional instructions for both 92227 and 92228 state:

  • Do not report 92227 in conjunction with 92002-92014, 92133, 92134, 92250, 92228 or with the evaluation and management of the single organ system, the eye, 99201-99350)

Other new codes replace the preceding Category III codes 0176T and 0177T. New level one codes are:

  • 66174 Transluminal dilation of aqueous outflow canal; without retention of device or stent
  • 66175 Transluminal dilation of aqueous outflow canal; with retention of device or stent

Two new CPT codes exist for the use of amniotic membrane along with a series of additional instructions and a revision to the existing ocular surface reconstruction code.

  • 65778 Placement of amniotic membrane on the ocular surface for wound healing; self-retaining
  • 65779 Placement of amniotic membrane on the ocular surface for wound healing; single layer, sutured
  • Do not report 65778, 65779 in conjunction with 65430, 65435, 65780
  • For placement of amniotic membrane using tissue glue, use 66999
  • 65780 Ocular surface reconstruction; amniotic membrane transplantation, multiple layers
  • For placement of amniotic membrane without reconstruction using self-retaining or single layer suture technique, 65778, 65779

The longstanding CPT code for laser iridotomy was revised in 2011 to read:

  • 66761 Iridotomy / iridectomy by laser surgery (eg, for glaucoma) (per session)

This CPT code was previously a “one or more sessions” code.

The new codes take effect January 1, 2011. Payment rates will not be known until the Medicare Physician Fee Schedule is published.
