Reimbursement for Diagnostic Tests with Topcon’s Aladdin (Topcon Medical Systems)

This monograph describes reimbursement for diagnostic tests performed with Topcon’s Aladdin optical biometry and topography system, either alone or as part of another service.

Topcon’s Aladdin optical biometry and topography system is a multipurpose diagnostic instrument for evaluating the anterior segment using optical coherence biometry, corneal topography, and other imaging.  The instrument can perform multiple tests in as little as 5 seconds depending on the patient’s and provider’s needs.  These tests include:

  • Optical coherence biometry
  • Full corneal topography by Placido rings
  • Anterior chamber depth
  • Lens thickness
  • Central corneal thickness
  • Keratometry
  • Zernike analysis of the cornea
  • White-to-White measurement
  • Static and dynamic pupillometry
  • IOL calculation, including post-refractive corneal surgery and multiple surgeon customization

These tests supplement the eye care professional’s evaluation and management of the cornea, cataracts, and planning for anterior segment surgery. Some features may additionally be useful for patient education.

The Aladdin provides the user with the ability to view, photograph and measure the anterior segment of the eye without direct contact with the cornea.

Much of the information in this document is taken from official publications of the Medicare program. The reader is encouraged to check with the local Medicare Administrative Contractor (MAC) for additional information and instructions. In the case of other third party payers, we have used the coding concepts contained in Current Procedural Terminology (CPT) and published by the American Medical Association (AMA); diagnosis codes are from the International Classification of Diseases (ICD), ICD-10-CM (10th edition). Documentation of a diagnostic test, and the medical rationale for it, are key to reimbursement so we describe the required elements in detail.

Since economic analyses are a necessary part of any capital budgeting decision, we incorporated Medicare’s payment rates for the procedures as well as utilization rates.
